1.  Please complete the entire form. Make checks payable to "Fayette Flyers".  New members bring it to the meeting.

2. Club membership fee is $50.00  for family.

3. A family is defined as those persons related by marriage, blood, or adoption and residing in the same residence.

  IT is requested that you attend at least your first meeting to be voted in.

Yearly Renewal dues will be  (Fifty dollars $50.00 Family membership).

All renewals must be paid by December 31^st.   A late fee of $10.00 will be added after January 31^st.  After March 31^st the member will be removed from the roster and next on the waiting list if any will be up to join..
